Dear New Patient:
Thank you for choosing Triangle Urology Associates, PA. All Patient Registration is done in our offices on a Phreesia Pad or online prior to your appointment (look for text and/or email giving you this option).
(Note: you must be at our office 30 minutes before your appointment time unless you have completed online registration- then you only need to be at the office 10 minutes early.)
Please bring the following items to your appointment:

Insurance card or cards.

Photo identification.

List of all your medications or bring all of your medications with you.

Copay.

Any payments due on old balances.

If you cannot keep this appointment give us a call at 919-477-7003 and we will reschedule the appointment for you.
If you have coverage through an HMO and need a referral to be seen please contact your primary care physician prior to your visit. You are responsible for bringing your current insurance card so that we can file the claim appropriately. Failure to bring your current insurance will result in you being responsible for the entire payment. For your convenience we do accept MasterCard, VISA, check or cash.
